This form and applicable deposits must be received in the ISV USA Office no later than 105 days prior to departure. Within 15 business days of receipt, ISV will provide a sample itinerary and quote of airfare. This will be included on your final invoice, which will be mailed 80 days prior to your departure date. All payments must be received in the ISV USA Office no later than 65 days prior to departure. If your full airfare payment is not received on or before 65 days prior to departure, your reserved space on the ISV Group Flight will be released.

FLIGHT OPTIONS
All Custom Program participants are required to take a flight organized by ISV.
A $100.00CAD non-refundable deposit is required for each country visited.

Note: Due to the variables of ISV Custom Programs in multiple countries, the cost of airfare cannot be guaranteed at this time. ISV will provide a sample itinerary and quote of airfare within 15 business days of receipt of the form and applicable deposits.

COSTA RICA DEPARTURE CITY FLIGHT OPTIONS
I would like ISV to schedule my Lacsa/Taca Airline flight to depart for Costa Rica from the city checked below. To add the Spanish Language & Latin Dance Lessons Package, please check option below. Note: ISV will do everything to accommodate your request, however, it may be necessary to depart or arrive into a different city based on your Custom Program allocation.
