ALL PARTICIPANTS MUST COMPLETE AND SUBMIT THE FLIGHT OPTIONS FORM EVEN IF YOU ARE NOT TAKING THE ISV GROUP FLIGHT. Note: Custom Program participants must complete and submit a Custom Flight Options Form located at:

This form and applicable deposits must be received in the ISV USA Office no later than 105 days prior to departure (See address below). Your airfare will be included on your final invoice, which will be mailed 80 days prior to your departure date. All payments must be received in the ISV USA Office no later than 65 days prior to departure. If your full airfare payment is not received on or before 65 days prior to departure, your reserved space on the ISV Group Flight will be released.

Option #1:  I would like to reserve my seat on the American Airlines ISV Group Flight departing from Miami, Florida.
Standard 4-Week Program Group Flight. 

Option #2:  I would like to reserve my seat on the American Airlines ISV Group Flight and participate in the Spanish Language & Latin Dance Lessons at an additional cost of $370.00USD – departing five days prior to the ISV Program from Miami, Florida. 
Standard 4-Week Program Group Flight. 

Option #3: I will arrange my own flight to the Dominican Republic.

I understand that I am responsible for booking my flight correctly according to the ISV requirements indicated at http://www.isvonline.com/flightdrcanflightoptions.html.  I also understand I must provide the ISV USA Office with my personal flight itinerary via isvcan@isvonline.com at least 30 days prior to my departure and realize that this is only for ISV records and will not be checked for accuracy.

Note: Flight costs and taxes are subject to change due to fluctuating fuel surcharges, etc. implemented by the airlines. Prices may increase by as much as, but not more than $100.00 CAD.
